  • MODULE 1 - Introduction to the Various Types of Diabetes: Definitions & Guidelines

    This module includes topics such as the basic definition of diabetes, signs and symptoms of diabetes, acute hyperglycemia and major acute complications, how diabetes is diagnosed, epidemiology of diabetes disease state, and the clinical practice guidelines.

  • MODULE 2 - Culturally Appropriate Diabetes Support Across the Life Span: Overcoming the Barriers

    This module includes the role culturally appropriate care plays in providing support to people living with diabetes across the lifespan, and how to overcome cultural barriers to self-care and behavioral change.

  • MODULE 3 - Facilitating Behavioral Change to Support Self-Management: Create the Learning Environment

    This module includes the role culturally appropriate care plays in providing support to people living with diabetes across the lifespan, and how to overcome cultural barriers to self-care and behavioral change.

  • MODULE 4 - Self-Management Education: Making Real Life Recommendations

    This module includes topics such as self-management education and examines principles of healthy eating, being active, monitoring, taking medication, and reducing risks.

  • MODULE 5 - Successful Self-Management of Diabetes: Improving Access and Coordinating Care

    This module includes topics such as program management and business management.

  • MODULE 6 - Social Determinants of Health: Supporting Healthy Communities and Equal Access to Quality Healthcare

    This module includes topics such as community welfare and health equity, access to needed resources. Goals of Healthy 2020.
